How to Request an Assessment:
The special education evaluation process begins with a referral for determination of eligibility for special education services, and referrals can be submitted by caregivers, legal guardians, and educational personnel. If you’re interested in a special education evaluation for your child, please refer to the following steps.
1 - Prepare an e-mail or physical letter requesting a special education evaluation for your child. This letter can be typed or handwritten, and it should be sent to the Special Education Department for processing.
2 - Include the date, your child’s full name, your name, the reasons why you’re requesting an evaluation (this helps focus the assessment), and your contact information.
3 - Submit your written request to the LPS Special Education Department through email, US mail, or hand delivery. Following is the contact information.
