High School Athletics Requirement Overview (Grades 9–12)
To participate in Lynn Public Schools high school athletics, students and parents/guardians must complete the following requirements HERE: Final Forms Registration
- Contact Information
- Parent/guardian and student contact information, emergency contacts, and demographic information.
- Health History & Medical Profile
- Medical history, allergies, chronic conditions, and important health information used by the school nurse and athletic trainer.
- Medications
- List any medications the student currently takes while participating in athletics.
- Injuries & Hospitalizations
- Report previous injuries, surgeries, hospitalizations, or other significant medical history.
- Past & Ongoing Health Conditions
- Disclose any medical conditions that could affect athletic participation (such as asthma, diabetes, heart conditions, etc.).
- Emergency Medical Authorization
- Gives Lynn Public Schools permission to obtain emergency medical treatment if a parent/guardian cannot be reached.
- MIAA Pre-Participation Physical Evaluation
- Upload a current physical examination completed within the past 13 months, as required by the MIAA.
- Head Injury/Concussion Reporting
- Report any previous concussions or head injuries.
- Massachusetts Concussion Law Acknowledgement
- Parent and student review Massachusetts concussion regulations.
- NFHS Concussion Training Course
- Parent and student complete the required free NFHS online concussion course and upload the certificate.

- Equipment & Uniform Responsibilities
- Agreement to properly care for and return school-issued equipment and uniforms.
- Transportation Acknowledgement
- Review expectations for transportation to and from athletic contests.
- Eligibility, Attendance & Behavior
- Acknowledge academic eligibility requirements, attendance expectations, and student-athlete code of conduct.
- Transfer Policy
- Review MIAA and Lynn Public Schools transfer eligibility rules.
- Media Release
- Permission regarding photographs, videos, and publicity involving student-athletes.
- Anti-Hazing & Bullying Law
- Review Massachusetts laws prohibiting hazing and bullying.
- Massachusetts Opioid Law
- Review required information regarding opioid misuse and pain medication.
